To move through the process as quickly and efficiently as possible,
please fax, mail, or drop off copies of the documents listed below. Note
that if you wish to have copies made, there is a charge of $5.
Once your paperwork has been received, you will be scheduled for a free Foreclosure
Prevention individual session.
If you have not received a modification and wish to receive assistance
with the process, please submit the following:

- Previous year's Federal tax return (second page of form 1040 must
be signed and dated) and W2s
- Three
months of pay stubs for each member of the household over the age of
18
- Evidence of
any additional income, such as SSI, SSA or Retirement Award
letters
- If
you are self-employed, 3
months Profit-and-Loss statement for your business, signed and
dated
- Three months
of bank statements
- Hardship letter
- a
letter which describes your situation and
the reasons for which you may or already have become
delinquent on your mortgage, signed and dated.
- Latest mortgage
statement for all loans on the property
- Copy of your credit
report from one credit reporting bureau. Get a free credit report from
www.annualcreditreport.com.
- If you have Homeowners' Association, submit latest HOA
statement
- Utility bill
ATTENTION: If you
received a
modification or the Making Home Affordable Trial/Forbearance
Agreement and your lender requires you to receive counseling from a
HUD-Approved Agency, please submit copies
of the following documents:
- Intake
